If you are being admitted into the Oasis of Hope Hospital, please bring with you the following medical records. Please make sure to bring only copies, not original documents:

1. History and physical performed within the last 12 months. Also include: chemotherapy treatment charts, if applicable, as well as any other records pertaining to treatments received (radiation therapy, hormones, etc.) and currently prescribed medications.
2. CBC (Complete Blood Count) and recent Comprehensive Metabolic panel (chemistry-plus) blood tests and/or tumor markers, dated no more than two weeks before.
3. Copy of the pathology reports (either biopsy or post-operative).
4. Copy of written report of CAT scans, MRI’s, X-Rays, Ultrasounds, PET scans, etc.
5. Copy of the operative procedure report, if surgery was performed, and discharge summary.
6. Bring films (CT scans, MRI’s, X-Rays, Ultrasound, PET scan, etc.) If possible, try to get these recorded on a CD.

NOTE:

1. All copies of medical records will become property of Oasis Hospital.
2. All X-Rays, CT Scans & MRI films will be returned to you before you leave.
